Online Registration Summer 2010

Instructions for registering & paying online.

  1. Please chose the course/courses you want to purchase and complete the secure Pay Pal process.
  2. After completing payment, Click on “Return to Merchant” link on the lower left-hand corner.  You will be asked to download the Online Consent PDF form. (Can't read the PDF? Get the free download of Adobe Reader here.)
  3. Print out the form, fill it out and please sign the consent waiver!
  4. Please mail it to:

  5. Please check the box to share your phone # with DaVinci Experience in case we have questions about your registration and need to reach you.

We can not consider your registration complete until
we receive the Online Emergency Medical / Contact Form by regular mail.

Summer Champs participants accepted at all sites and do not need to enclose registration fee. United Way will be billed.
Write your Summer Champs ID # on your registration form and send it by regular mail to the address above.

Falmouth Congregational Church Site

Reduced Price when registering for 3 weeks of 9am-3pm camp.
Choices of Half day camp 9am-noon for 4-7 yr. olds, full day 9am-3pm for 5-13 yr. olds, and AM extension starting at 7:30 am and/or PM extension until 5:30 pm. Five, six and seven year olds have a choice of half day or full day sessions most weeks.

The themes of The Write Stuff and Emergency Vet are only offered as full day sessions 9am-3pm (with the option of extended day available).

The $50 Registration Fee per week per child included in your weekly tuition fee is Nonrefundable. Cancellations must be made at least 3 weeks prior to the start of a session to qualify for a tuition balance refund.
